Thank You to Thomas Cook (FSU’s October 2015 Guest Speaker)


Families Speaking Up would like to thank Thomas Cook for taking the time to offer his guidance in navigating the person-centered planning process. It is imperative to understand it as your child ages out of schooling and enters the adult world.

Thomas Cook, Executive Director of the Delaware Association of Rehabilitation Facilities, reviewed the federal requirements for person-centered planning and discussed a process for determining “the most integrated setting appropriate” for our family members. Mr. Cook has had experience in working with person-centered planning in several states, including the state of Minnesota, where he designed the person-centered planning component for the Medicaid assessment for long term services and supports with consultation from Beth Mount, the originator of personal futures planning.
